Temperament Questionnaire
This questionnaire identifies a person's inborn, God-given temperament. Each of you will receive an email with access to these online questionnaires. These questionnaires must be completed prior to attending the Engaged Couples Temperament Retreat.
PREPARE Inventory
The Basilica uses a marriage preparation inventory called PREPARE. All couples to be married in this Archdiocese, and in many other Dioceses throughout the country, are required to complete a pre-marriage inventory and review the results with trained staff. This tool is designed to encourage conversations about your relationship and backgrounds, and to address important issues prior to your marriage.
Each of you will receive an email confirming your registration for this Inventory. Please watch for this email with a link to the online survey and log-in code. This tool is designed to encourage conversations about your relationship and background, and to address important issues prior to your marriage. This Inventory forms the basis for the Inventory Review Workshop with Marriage Mentor couples from The Basilica. Allow about 45 minutes to complete the questionnaire.
Letter Requesting Copy of Baptism Records
If you are Catholic, please obtain a newly issued certificate of baptism from the church where you were baptized. This must be issued within six months of your wedding date. If you are baptised in another faith tradition, please send The Basilica a copy of your original baptismal certificate, or request a copy from your church of baptism. The Basilica's maillng address is on this form if the church perfers to mail it directly to us.
Freedom to Marry Form
Each of you will be asked to have a parent, or another person who has known you well throughout your life, complete this form.
Visiting Presider Form
Please use this form if you have made arrangements to have an outside Catholic priest or deacon, or minister of another denomination in addition to the Catholic officiant, preside over your wedding. Please submit this form as soon as the arrangments are made. The Basilica will follow up with your officiant.
Reduced Marriage License Form
Minnesota state law allowed for a reduced marriage license fee when the couple has complted 12 hours of premarital education. You can either mail it to The Basilica in one of the self-addressed return envelopes, or email it to the marriage office. Please allow five business days for processing.
